A Twist of Faith Outreach is a Community Health Ministry dedicated to strengthening individuals, families, and communities through resource navigation, strategic partnerships, and community-based support.
We connect people to resources and services that promote health, hope, and stability while collaborating with churches, healthcare organizations, nonprofits, and community leaders to expand access and strengthen community well-being.
Through compassionate service and collaborative partnerships, we are committed to building healthier communities through faith and action.

Resource Navigation
We connect individuals and families to trusted resources, services, and support that promote health, hope, and stability. Through personalized guidance and referrals, we help community members identify and access the resources they need.
Community Outreach & Engagement
Through outreach initiatives, community events, and collaborative service opportunities, we meet people where they are, build relationships, and connect them to resources and support.
Community Resources & Support
Through partnerships and resource-sharing initiatives, we help increase access to essential items, supportive services, and community-based assistance that address immediate needs and strengthen overall well-being.
Strategic Partnerships
We collaborate with churches, nonprofits, healthcare organizations, community leaders, and service providers to expand access to resources, strengthen community networks, and create lasting impact.

Faith in Action (CoP)
A Community of Practice for Collective Impact
Faith in Action (CoP) is a Community of Practice that connects faith-based organizations, nonprofits, healthcare providers, community leaders, and other partners who share a commitment to strengthening communities and improving community well-being.
Through collaboration, shared learning, resource exchange, and strategic partnerships, members work together to address community needs, strengthen organizations, and expand access to resources and services.
Together, we leverage our collective knowledge, expertise, and relationships to create sustainable solutions and build healthier communities through faith and action.
